Hospice?

Hospice provides expert, coordinated care for people facing serious illness. Our team works closely with you, your loved ones, and your healthcare providers to honor your goals and support your physical, emotional, and spiritual well-being.

Hospice Services

St. Francis Hospice provides wraparound services for both patients and families. We meet you where you are and brings care that is aligned with your medical, emotional, and spiritual needs.

Medical Care

Your care team will come to you, whether you are in your home, in our inpatient unit, or in a facility. We are also available through the phone 24/7.

Emotional & Spiritual Care

Navigate this chapter of life with a support team that focuses on your emotional and spiritual needs.

Family Care

Families and caregivers receive support as well. Joining hospice eases the load for caregivers. Services continue after their loved one passes.

We offer respite services at no cost for up to 5 days at our inpatient unit in Nuʻuanu or in a nursing home.

Meet Your
St. Francis Care Team

Our hospice care team is made up of a group of professionals and volunteers who work together to care for patients and families—addressing their physical, emotional, and spiritual needs. Our interdisciplinary hospice team includes:

Hospice Physician/APRN

Oversees care plans, manages complex symptoms, and consults with patients’ primary doctors.

Social Worker

Supports emotional well-being, assists with practical needs (financial, legal, or care planning), and helps families navigate end-of-life transitions.

RN/LPN

Provides direct medical care, symptom management, medication administration, and education for families.

Chaplain/Spiritual Counselor

Offers spiritual care and guidance, regardless of faith tradition or belief system.

CNA/NA

Helps with personal care (bathing, dressing, grooming, feeding) and provides comfort to the patient.

Volunteer

Offer companionship, respite for caregivers or share special skills (music, art, pet therapy, etc.).

Navigating Grief Together

Our bereavement social workers walk with you on your journey for a year following the passing of your loved one. You also have unlimited access to our grief support groups, grief cafes, and our twice-yearly memorial events.
