Client's Photo ID (required),
Client's COVID-19 vaccination card (required), and
Other legal documents (i.e., Advance Healthcare Directive, POLST)
Finally, the client's physician must also complete the following form:
We recommend having the client's physician complete this form prior to beginning your application as you will not be able to save your progress on this application.
