Thank you for your interest in St. Francis Adult Day Center at our ʻEwa Villages or Liliha campus. We offer a comfortable and secure group setting for older adults who may need some assistance and companionship during the day. This online application includes the following:
In addition to completing the above documentation, we also require the following attachments:
Participant's Photo ID (required),
Participant's COVID-19 vaccination card (required), and
Other legal documents (i.e., Advance Healthcare Directive, POLST)
Following review of your application, we will contact you to set up a tour.
Once Admission Agreement and Application form have been completed, the Program Manager will reach out to you and provide you with additional forms such as Health and Physical Form and TB form for 2-step PPD Skin test, QuantiFERON TB Gold Plus or Chest Xray as a state licensing requirement for admissions into ADC Program.
