St. Francis Community Health Services

The Tree of Life is located in the lobbies of the Maureen Keleher Center and Maurice J. Sullivan Family Hospice Center.
St. Francis Community Health Services is an umbrella of services providing statewide care on an outreach basis. It includes St. Francis Home Care on Oahu and Kauai; St. Francis Hospice with two residential facilities and hospice home care; Health Services for Senior Citizens (bathing service), Lifeline personal emergency response system, and St. Francis Parish Nurse/Community Outreach Program. St. Francis Community Health Services also co-sponsors two Mobile Care Vans, which provide dental services to residents on the Island of Hawaii.




St. Francis Hospice
Hospice care helps improve the quality of life of terminally ill patients and their families.

St. Francis Hospice is the oldest and largest hospital-based hospice program in the State of Hawaii. It was created in 1978 to ease the physical discomfort of terminally ill patients and allow them to face "death with dignity." Hospice patients must have an attending physician who believes the patient has a life expectancy of six months or less and must have completed curative treatment. Bereavement support and respite care are provided to family members.

The 12-bed Sister Maureen Keleher Center is located in Nuuanu.

St. Francis Home Care
Home Care offers a comprehensive range of services in the comfort of your own home.

Since 1962, St. Francis Home Care has offered an effective, less costly alternative for patients who need post-hospitalization care and rehabilitation services. Patients receive these services in the comfort of their own home. Home Care services include:

Care by nurses or home health aides
Occupational therapy
Medical social services
Laboratory services
Physical therapy
Speech language therapy
Nutrition counseling and assistance

Home care costs are generally covered - fully or in part - by insurance, including Medicare, Medicaid, HMSA and other programs that may require prior authorization. Physician referral is required.

Health Services for Senior Citizens
St. Francis Health Services for Senior Citizens, in collaboration with the Elderly Affairs Division, Department of Community Services of the City and County of Honolulu, offers a personal bathing service for those individuals over the age of 60 who qualify under the Federal, State and City Guidelines.

Lifeline
St. Francis was the first to bring Lifeline, a personal emergency response service, to Hawaii in 1983. The service assures frail elderly and individuals with chronic medical conditions living at home receive help at the press of a button, 24 hours a day, 365 days a year. Within seconds, a certified Lifeline professional responds, assesses the situation and summons appropriate help. A new feature also reminds clients when to take medications.

Mobile Care Health Project
The Mobile Care Health Project brings dental care to those in need on the Big Island.

The Mobile Care Health Project is a collaboration between the Office for Social Ministry (Diocese of Honolulu) and the St. Francis Healthcare System. This program provides dental care services to those who are unable to access or afford this care in the North and West Hawaii communities. Services are provided through mobile vans that are outfitted with dental equipment.

St. Francis Parish Nurse/Community Outreach Program
Begun in 1983, the program brings health education and promotion of healthy lifestyles to parishes and their surrounding communities. St. Francis Parish Nurses empower individuals to take an active role in the healing ministry of Christ.
