“I was glad to have my mom back at home after her hospital stay. But now what? I don’t know how to take care of her and I don’t want her to wind up back in the hospital.”
Taking care of an aging or chronically ill family member can be rough on you and others in your family. Your doctor may suggest home health to help make sure loved ones get the care they need. Their doctor might initiate these services after an office visit or when being discharged from the hospital. St. Francis Home Care Services has the resources and programs to support you in providing excellent care with less stress.
Bringing quality medical care right to your home
St. Francis Home Care Services helps patients recover after being discharged from the hospital or after an illness at home. Our goal is to help get them back on their feet as quickly as possible. We recognize that most people, given a choice, would prefer to receive care at home and avoid emergency room visits and admissions to the hospital. St. Francis Home Care Services is accredited by The Joint Commission, and our team of nurses, therapists, home health aides, and other professionals on Oahu and Kauai are ready to provide specialized medical care to help you or your loved one recover faster in the comfort of your own home.
Understanding that people often recover faster in their own comfortable surroundings, St. Francis was the first to introduce home health services to Hawaii in 1962. In 1977, services were expanded to Kauai.
St. Francis Home Care Services focuses on your optimum well-being, bringing a range of treatment and technology into your home when you need post-hospital care or rehabilitative services. Our goal is to help you recover fully and return to your everyday activities as soon as possible. Your doctor will determine which services you need at home and will continue to supervise your care. Together, your doctor and our home health professionals will develop a personalized plan of care for you. We also encourage family members to be involved in your care and recovery as much as possible.
State-of-the-art telehealth monitoring
St. Francis Home Care Services also offers telehealth service, which is ideal for patients with chronic illnesses such as congestive heart failure, chronic obstructive pulmonary disease or diabetes. The telehealth equipment is set up in the home and patients use it to monitor their vital signs such as weight, blood pressure, blood sugar level or oxygen in the blood. Telehealth does not replace visits by nurses, but allows them to monitor patients remotely on a daily basis and provide immediate attention if a patient’s medical condition is changing.
