There’s no place like home when you are surrounded by love and everything that’s familiar, and this is especially true when you’re recovering from an illness or injury. Technological advancements have made it possible for more treatments to be done safely and effectively right in your own home.
Understanding that people often recover faster in their own comfortable surroundings, St. Francis was the first to introduce home health services to Hawaii in 1962. In 1977, services were expanded to Kauai. St. Francis Home Care Services focuses on your optimum well-being, bringing a range of treatment and technology into your home when you need post-hospital care or rehabilitative services. Our goal is to help you recover fully and return to your everyday activities as soon as possible.
Your physician will determine which services you need at home and will continue to supervise your care. Together, your physician and the home care professionals will develop a plan of care for you. It’s important that you understand and can maintain your individualized course of treatment between home visits. We also encourage family members to be involved in your care and recovery as much as possible.
Home health services, ranging from skilled nursing care, personal care and therapies, to medical social services, are provided by a caring team of health care professionals. To qualify for home health services, you must be homebound or unable to leave home unassisted and need at least one of the qualifying services:
· Skilled Nursing Care
Registered nurses provide health education and assessment in addition to services such as changing wound dressings and various tubes, intravenous (IV) therapy, respiratory care, and instructions to the patient and family/caregiver.
· Physical Therapy
Physical therapists assist with rehabilitation after an illness or injury. They work with you in developing an exercise plan to regain movement and strength in the body and provide training on how to use special equipment or do daily activities. Home assessments are made for safety and equipments needs.
· Speech/Language Therapy
Speech/language pathologists help with exercises to regain and strengthen speech skills.
Additional services may be provided once you are admitted for home care services:
· Home Health Aides
Certified home health aides provide personal care services such as bathing and dressing and assist nurses and therapists with treatment programs.
· Occupational Therapy
Occupational therapists will work with you to accomplish activities of daily living (ADL) by yourself, such as feeding, dressing and bathing, as well as learn new ways to do activities.
· Medical Social Services
A medical social worker helps you with social and emotional concerns related to your illness or injury. This may include counseling or finding resources in the community.
· Nutritional Counseling
A dietitian helps families plan healthy meals or special diets for recovery.
Most home health costs, including services and certain medical supplies except prescription drugs, are covered in full or in part by Medicaid, Medicare, HMSA or other private insurance programs. St. Francis Home Care Services is a Medicare-certified Home Health Agency, nationally accredited by The Joint Commission. Accreditation confirms our commitment to quality service and excellence.
